Full-time Opioid Health Home (OHH) Case Manager functioning as the central point of contact for coordinating patient-centered care across the core services provided at GLRC. Beneficiaries will work with an interdisciplinary team of providers which will develop and coordinate an individualized recovery care plan to best manage their whole person care.
Minimum requirements include a Bachelor’s Degree with 3 to 5 years’ experience providing services in counseling or human service related role. Additionally, CADC certifications and/or Master’s Degree may prove beneficial. Previous Case Management experience helpful. Characteristics include strong written and oral communication abilities, excellent case conceptualization skills, strong organizational qualities, and strong leadership and team participation required.
