Community Living Supports (CLS) are used to increase or maintain personal self-sufficiency, facilitating an individual’s achievement of his/her goals of community inclusion and participation, independence or productivity. The supports may be provided in the participant’s residence or in community settings (including, but not limited to, libraries, city pools, camps, etc.). Examples of CLS services that can be provided include, meal preparation, laundry, routine seasonal and heavy household care and maintenance, money management, and socialization and relationship building.
Essential Functions
• Attend and participate in the Person Centered Planning (PCP) process when invited.
• Implement goals identified in the Person Centered Plans (PCP) and other plans a participant may have by providing assistance, reminders, observation, guiding and/or training.
• Record Service logs each time a service is provided to a participant. Complete other paperwork as requested by the residential supervisor and turn in within required timeframes.
• Provide a culture of gentleness, dignity, respect, and a safe environment to all participants receiving Community Living Supports.
• Assure all participants are aware of their rights.
• Attend all required meetings, including staff meeting and training/in-services as scheduled.
