Eisenhower Center is seeking a motivated and dynamic individual to provide Program Coordinator services at our residential rehabilitation facility serving residents that have suffered a traumatic brain injury or other related health issues. Candidates must have a Bachelor's degree in a related field, but a Master’s degree in fields such as Social Work and Counseling are preferred. Strong communications skills are required. Candidates must have experience in post-acute rehabilitation program management. Qualifications:
•Bachelor’s Degree
•Valid Michigan driver's license and be insurable
•Knowledge and/or experience of all Microsoft office systems
•Must have 5 years of knowledge and/or experience in rehabilitation and behavioral programming.
•Team oriented
•Impeccable written and oral communication skills.
Duties and Responsibilities:
•Performs case management functions within a post-acute residential program.
•Ensure successful rehabilitation outcomes through ongoing program evaluation and client advocacy.
•Maintains positive family, referral source, and funder relations.
•Actively seeks and develops referrals to the program through networking.
•Facilitates client’s staffing and team meetings with clinical staff and rehabilitation team.
•Participate in management on-call rotation
•Other duties as assigned
