Description COVENANT HEALTHCARE US:MI:SAGINAW VARIABLE SHIFT FULL TIME BENEFITED Summary: THE CLINICAL OUTCOME SPECIALIST - CARDIOVASCULAR SERVICES (CONS-CVS) DEMONSTRATES EXCELLENT CUSTOMER SERVICE PERFORMANCE IN THAT HIS/HER ATTITUDE AND ACTIONS ARE AT ALL TIMES CONSISTENT WITH THE STANDARDS CONTAINED IN THE VISION, MISSION AND VALUES OF COVENANT AND THE COMMITMENT TO EXTRAORDINARY CARE FOR EVERY GENERATION. THIS INDIVIDUAL WILL COORDINATE CLINICAL, QUALITY, EDUCATIONAL (INTERNAL AND EXTERNAL) AND FINANCIAL ACTIVITIES FOR THE PATIENTS, MEDICAL STAFF, NURSING AND SUPPORT SERVICES. THE CONS PROVIDES DIRECTION FOR CHANGES IN BEST NURSING PRACTICE BASED ON RESEARCH, ACTS AS A PROFESSIONAL ROLE MODEL AND CLINICAL CONSULTANT WHILE FUNCTIONING AS A SPECIAL PROJECT COORDINATOR. ADDITIONALLY, THIS PERSON MUST DEMONSTRATE EXCELLENT COMMUNICATION SKILLS, LEADERSHIP, DECISION-MAKING ABILITIES, ADVANCED KNOWLEDGE AND CLINICAL SKILLS IN THE CV SPECIALTY. THIS POSITION REQUIRES A MOTIVATED, SELF-STARTER WITH CRITICAL THINKING ABILITY AND ABILITY TO WORK AN INDEPENDENT ENVIRONMENT WITH A FLEXIBLE WORK SCHEDULE TO MEET PROGRAMMATIC NEEDS. Responsibilities: CONTRIBUTES TO THE ORGANIZATIONAL SUCCESS PILLARS OF SUCCESS INCLUDING, FINANCIAL AND QUALITY PERFORMANCE, PATIENT EXPERIENCE AND EMPLOYEE ENGAGEMENT. COORDINATES ALL STRUCTURAL HEART ACTIVITIES (TAVR/WATCHMEN/MITRAL CLIP, ETC): COMMUNICATES AND COORDINATES SCHEDULES AND PROVIDES PATIENT EDUCATION AND ASSIST WITH PATIENT FLOW ON DAY OF PROCEDURES. ADDITIONALLY, COORDINATES STRUCTURAL HEART TEAM MEETINGS/CONFERENCES AND WORKS DIRECTLY WITH MEDICAL DIRECTOR, PHYSICIANS AND STAFF AS APPROPRIATE. PROVIDES LEADERSHIP TO THE HEALTH CARE TEAM TO DEVELOP, IMPLEMENT AND EVALUATE NEW PATIENT CARE PROGRAMS AND OUTCOME GOALS SPECIFIC TO THE PATIENT POPULATION. ANALYZES PRACTICE AND DESIGNS MULTIDISCIPLINARY INTERVENTIONS FOR IMPLEMENTATION AND TESTING TOWARD ACHIEVEMENT OF EVIDENCED BASED OR BEST PRACTICE FOR PATIENT POPULATION. COLLABORATES WITH THE HEALTH CARE TEAM TO DEVELOP CARE METHODS (PLAN OF CARE, ALGORITHMS, PROTOCOLS, ORDER SETS, CLINICAL PRACTICE GUIDELINES) FOR SPECIFIC PATIENT POPULATIONS AND FOR CV AND PATIENT SERVICES. EVALUATES PATIENT CARE PRACTICES, INTEGRATING RESEARCH-BASED THERAPEUTICS AND CONTINUOUS QUALITY IMPROVEMENT TO ENHANCE OUTCOMES. WRITES/REVISES GUIDELINES AND PRACTICE POLICY BASED ON RESEARCH. RECOMMENDS PROPOSED THERAPEUTICS (DRUGS, EQUIPMENT, PRODUCTS) BASED ON AN ANALYSIS OF EVIDENCE-BASED PRACTICE, COST-EFFECTIVENESS, CLINICAL EFFICACY, AND EFFICIENCY. COORDINATES, DEVELOPS AND EVALUATES PATIENT AND FAMILY EDUCATION FOR EFFECTIVENESS AND CULTURAL APPROPRIATENESS. COMMUNICATES WITH PATIENTS AND FAMILIES IN A PROFESSIONAL, THERAPEUTIC, AND COURTEOUS MANNER. PROVIDES CLINICAL EDUCATION IN-SERVICE AND COMPETENCY TRAINING FOR STAFF AS REQUESTED BY CV MANAGERS/COORDINATORS. PARTICIPATES IN PATIENT CARE AND UNIT ACTIVITIES TO ACHIEVE COMPLIANCE WITH ALL REGULATORY REQUIREMENTS (HFAP, CMS, THIRD PARTY INSURERS, ETC.) SERVES AS CVNS REPRESENTATIVE AT PATIENT SERVICES, NURSING QUALITY COUNCIL, EPIC, CQPVA, HFAP, DOCUMENT REVIEW COMMITTEE AND ADMINISTRATIVE, DEPARTMENT AND QUALITY COMMITTEES AS ASSIGNED OR REQUESTED. COORDINATES, DEVELOPS, ANALYZE AND EVALUATES QUALITY INTERVENTIONS AND OUTCOMES FOR CV SERVICES. ABSTRACTS DATA THROUGH CONCURRENT AND/OR RETROSPECTIVE MEDICAL RECORD REVIEW. REVIEWS DATA FOR TRENDS AND IDENTIFICATION OF POTENTIAL CAUSATIVE FACTORS. SUBMITS DATA TO APPROPRIATE DATABASE AS INDICATED. MAINTAINS TAVR AND DOOR-TO-BALLOON REGISTRY AND OTHER REGISTRIES AS ASSIGNED. SERVES AS A CV SERVICE LINE LIAISON AND WORKING COLLABORATIVELY WITH LEADERSHIP, PHYSICIANS, NURSING AND SUPPORT STAFF; AS WELL AS MULTIPLE DEPARTMENTS (ECC, CVICU, ETC.) THROUGHOUT THE ORGANIZATION. ANNUAL REVIEW OF CV POLICY AND PROCEDURES, ORDER SET, ETC. SERVES AS SPOKESPERSON FOR CARDIOVASCULAR SERVICES, STRUCTURAL HEART PROGRAM, HEART DISEASE AND HEALTHY LIFESTYLE-WILL BE CALLED UPON TO MEET WITH LOCAL PRESS, TV AND COMMUNITY OUTREACH ACTIVITIES (HEALTH FAIRS, ETC.). MAINTAINS PROFESSIONAL DEVELOPMENT THROUGH SEMINARS, WORKSHOPS, IN-SERVICE ACTIVITIES, AND CURRENT PROFESSIONAL LITERATURE. OTHER DUTIES AS ASSIGNED.
