GENERAL PURPOSE OF THE JOB
This position drives growth and clinical outcomes for PD and HHD. The HDL is the subject-matter expert on home initiatives in the division. The HDL partners with the operators (RODs, GFAs, Home Regional Leads/HPMs, FAs) and CSS to co-manage growth performance and clinical outcomes.
REPORTING STRUCTURE
•Position reports directly to DVP
•Equivalent to Group-FA level position
•HDL typically has home experience, but HDLs with strong in-center experience can perform highly in role as well
•Officially reporting through field operations organization with 'dotted line' reporting to the Home Group Lead and Home DCS
•Home Regional leads and/or Home Program Managers indirectly report to the HDL
ROLES and RESPONSIBILITIES
•Growth
•Drive PD conversions through on-site visits, educational events, and other promotional activities at in-center units across division
•Deep dive problematic in-center units ("black holes") to build effective conversion process
•Conduct quarterly "home champ" or mixed-modality FA training on how to manage / drive home performance in their in-center programs
•Drive planned start growth by working with RODs and DVP to educate supporting nephrologists and provide physician metrics
•Manage and review quarterly PD bonus incentives platform - the PD NPG bonus and divisional conversion bonus.
•Monitor and manage the weekly PD patient pipeline
•Clinical
•Partner with Home DCS, Home Regional Leads, and Home TMs to improve patient retention on home therapies
•Responsible for clinical outcomes related to Adequacy, Infection and Access management
•PTN - Responsible for leading monthly PTN audits
•PDC Complications - Facilitate conversation with local surgeons and nephrologists to deep-dive challenges with catheter placement and opportunity to strengthen learnings
•Ensures that divisional PD RNs are identifying high-risk-for-loss patients and taking proactive measure to stem loss
•Team
•Facilitate monthly call with divisional PD and HHD RNs to update on and reinforce current initiatives
•Lead and organize quarterly/bi-annual Home Summits for PD and HHD TMs to provide strategic updates, divisional performance, educational sessions, and support divisional nurse coherence
•Support and hold accountable RODS and FAs towards PD growth and clinical goals in the respective regions
•Provide strategic and tactical insights at regular core team and VSA strategy sessions
•Financial / Operations
•OpEx - Responsible for monitoring performance of PD and HHD supply expenses and supporting ROD in understanding drivers of budgetary overage
•Staffing - Monitor staffing ratios to ensure that home programs are properly staffed and hiring ahead for growth prospects
•Support ROD and FAs as needed in cases of Home program closure

•HDL expected to attend regular HDL lead meeting
•Weekly 1-hour calls with HDL team led by HGL and Home DCS
•Quarterly - live 2-day meeting with HDL team in Oak Brooke, IL
•Travel Required: up to 50%, mainly within the division by car to both ICHD and Home units
