Job Summary:
The Director of Care Management is accountable and responsible for the departments of Social Work, Utilization Management, and Care Management, (specific sites may have additional clinical areas of responsibility). The Care Management Director assures that patient care is facilitated, coordinated and integrated across the continuum in order to assure and enhance patient and family engagement. Additionally, this role assures that care coordination is outcomes focused and is managed in a cost-effective manner to decrease length of stay, manage readmissions and to effectively manage utilization review functions at the site.
Minimum Qualifications:
•Education: Bachelors of Science in Nursing Required. Masters degree in Nursing or Masters in related field Required.
•Licensure/Certification: Active and valid Michigan Registered Nurse Licensure. Certification in Case Management preferred
