The Social Worker, in collaboration with the interdisciplinary team (IDT), provides and facilitates acquisition of economic, social, and emotional support services to hospice patients and their families/caregivers. The Social Worker’s responsibilities include, but are not limited to: psychosocial consultation to other agency personnel; collaboration with other IDT members; referrals to outside agencies; homemaker services; and direct services to hospice patients and their families/caregivers.
Qualifications:
- Master's Degree in Social Work (MSW) obtained from a school of social work accredited by the Council of Social Work.
- Possession of a current license to practice social work issued by the state of Michigan. Licensed Masters Social Work (LMSW)
- A minimum of one (1) year paid medical Social Work experience required.
- Must be able to cope with family/caregiver emotional stress, cultural diversity and be non-judgmental of individual life-styles.
