Summary:
The Social Worker-BSW demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant and the commitment to Keeping our Promise of Caring. This individual is responsible for preparing social histories, treatment plans, progress notes, and discharge summaries for hospice and home care clients. The B.S.W. provides psychosocial intervention and resource management for these clients and assists with bereavement services under the supervision of a MSW.
Responsibilities:
Contributes to organization success targets for patient satisfaction and net operating margin
Assess the social/emotional needs of hospice patients/families upon request from physician, hospice/IDT members
Assess social, emotional, financial, spiritual and support system status for hospice and home care patients and families
Assess special needs related to culturally diversity and communication patterns
Assess for potential risk of suicide, fraud, and abuse
Assess environment resources and maintain safety
Assess caregiver s ability to function adequately
Assess need for counseling related to risk assessment for pathological grief
Assist with provision of bereavement services
Identify development level of patient/family obstacles to learning or ability to participate in the care of the patient
Identify obstacles to compliance
Evaluate for long term care when appropriate
Communicate psychosocial information to in-patient facility when level of care changes
Assist other caregivers (RN s, MSW) in explaining available community, hospice, or home care services
Evaluate patient/family response to intervention when referred to specific community resources
Provide short-term counseling, crisis intervention, assistance with advance directives, and funeral planning
When appropriate provide input regarding care of patients to the Skilled Nursing Manager, Skilled Nurse Development Coordinator, IDT Team Members, Hospice Manager, and/or Hospice Nurse Development Coordinator
Participate in area social service groups as indicated
Provide presentations to local community groups and staff on a variety of topics
Represents agency in a professional manner
Other information:
Education
Bachelor s degree in social work from an accredited institution
Skills
Demonstrates excellent customer service skills
Credentials
Current Michigan certification
Experience
Two years experience in a medical/health care related setting
Preferred: Home Care, Hospice
Knowledge of hospice and home care philosophy and services
Knowledge of existing community agencies and their functions
