This position is responsible for the coordination of day-to-day client activities. It is responsible for staffing and coordination of service requests for clients, assuring the conversion of all requests into staffed cases and the delivery of timely and quality services.
Primary Duties
* Assures delivery of service with both the client and caregiver.
* Works with various staff members to assign health care staff for new referrals and existing clients.
* Coordinates initial and on-going services in relation to client needs, satisfaction with service/caregiver, complaints, and altered/modified plan of treatment.
* Responsible for computer data entry in relation to scheduling.
* Inputs primary information into the computer system to generate client scheduling.
* Maintains timely changes in data base information for client schedules assuring the accuracy of the weekly schedule for billing and payroll purposes.
* Secondary intake for new referral information and scheduling.
* May take paraprofessional patient referrals via phone.
* If Private Duty RN requires assistance with volume of requests, may complete requests for paraprofessional services.
* Monitors employee hours.
* Monitors accumulation of employee hours to limit overtime scheduling as much as possible.
* Problem solves day-to-day issues to assure customer satisfaction.
* Performs other functions as assigned.
Job Requirements
* Strong human relations and telephone/communication skills a must
* Minimum of high school education required with prior experience scheduling in a home health care agency preferred
* Excellent organizational, interpersonal and problem solving skills
* Ability to work under time demands and pressures
* Current experience with or ability to learn a system managed computer network
* Ability to work well with others, a team player
* Willingness to participate in training and educational programs to benefit professional growth and promote relations with internal and external customers
