The Community Health Worker assists high-risk individuals and families through education, case management, and support related to maternal and early childhood health and wellness. This is a full-time, non-exempt, benefit eligible position. Typically working Monday - Friday, from 8:00am - 5:00pm.

Requirements:
- High school diploma, college coursework preferred.
- Minimum 2 years experience in a health care or community setting. Experience in an OB health care setting preferred, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job as listed above.
- African bilingual required.
Responsibilities:
- Meets with families in the home to clarify needs and progress toward goals
- In collaboration with families and MIHP staff, formulates a prioritized plan to meet needs
- Works collaboratively with clients and community agencies to schedule/provide services for families
- Documents services/education provided another pertinent program data
- Mentors, empowers and advocates for families to help them gain skills and independence
- Communicates and collaborates with families, program staff, referral agencies, and other professionals involved in the care of the client to enhance optimal coordination of services
- Acts as a resource to program staff about life/situational/cultural factors impacting the client, family and environment
- Apply families for Medicaid and Maternity Outpatient Medical Services and ensure continual coverage for families
- Case manage OB clients regarding Medicaid and Maternity Outpatient Medical Services
- Monitor and maintain referral lists, supplies, educational materials and equipment
- Coordinate patient care through scheduling and referrals for dental care, WIC, postpartum visits, well child checks, and immunizations
- Insure coordination of insurance and medical information regarding caseload among MIHP staff, medical records, medical providers, and the billing department
- Represent Cherry Street Health Services and Strong Beginnings at community events
- Adheres to universal precautions and CSHS policies.
- Understands and adheres to CSHS’s compliance standards as they appear in CSHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
- Keeps abreast of all pertinent federal, state and CSHS’s regulations, laws, and policies as they presently exist and as they change or are modified.
