Care Managers duties include but are not limited to:
- Keeping Field Managers up-to-date by reporting any significant client issues or problems
- Meeting with clients once weekly unless otherwise directed by insurance company or SAI management
- Attending and participating in client team meetings as scheduled by providers and/or case managers
- Communicating on a regular basis with client's team members such as residential facilities, psychiatrists, etc
- Attending TBI related physician appointments as scheduled. May also require to attend a non-TBI related appointment as needed
- Communicating effectively and professionally with external case managers as needed
- Communicating effectively and professionally with internal SAI co-workers
- Will often be called upon to assist clients in enhancing community integration skills by taking them out into the community
- Asking any questions and bringing concerns to Field Manager
- Regularly attending monthly staff meetings. If Care Manager cannot attend they are to notify Field Manager before the meeting and arrange to meet with Field Manager after the meeting for updates
- Remaining familiar with SAI policies and procedures as presented in the employee policies and procedures manual
- Completing client Advocate documentation at least weekly per policy
- Submitting any client reimbursement to the SAI office weekly
- Submitting mileage reimbursements to the SAI office monthly
- Completing Care Manager Monthly reports by the 7th working day of the following month per policy
