Key Responsibilities
The Clinical Lead will work in tandem with the Regional Practice Manager within the Practice Dyad to oversee the providers in the Practice. The Clinical Lead will be an expert within Physician Services EMR, internal operating systems and resources to achieve the quadruple aim. He/she will assist the Practice in delivering service where patients, families, referral sources and payor can “expect exceptional”. In addition to the duties above, the Clinical Lead will also provide primary and/or specialty medical care in a variety of assisted living, home, sub-acute rehabilitation and long term care settings.
In this role you will be responsible for:
- Expert within Physician Services EMR, internal operating systems, and resources.
- Resource for providers regarding nuances of different visit types, visit notes and care settings.
- Follows established service line rules and pathways.
- Shared responsibility with the Regional Practice Manager for productivity of the Practice provider group.
- Develop orientation plans in conjunction with Regional Practice Manager for newly hired providers.
- Oversee orientation for newly hired providers in conjunction with Regional Practice Manager.
- Assist Regional Practice Manager in facility relationship management to grow practice or assist with service recovery when needed.
- Provides steps to advance Practice Practitioners or works within the scope of the collaborative/Practice Agreement
- Ensures that organizational initiatives are implemented in a timely way throughout the Practice and that the Practice Dyad integrates improvement goals and programs into operations.
Required Qualifications
- State certification and degrees need to practice medicine as a Nurse Practitioner
- Current State License as a Nurse Practitioner
- 2 years clinical experience in a post-acute care setting
- Ability to practice medicine skills as defined by accepted licensures boards of Nurse Practitioner
- Demonstrates ability to assess and respond to the needs of patients, families and caregivers in post-acute care settings
- Demonstrates skill in practice of medicine, ability to cope with family emotional stress and tolerance of individual lifestyles
- Ability to handle multiple priorities, documentation requirements and deadlines
