Responsibilities/Job Functions
Perform in home care towards the client's therapy plan in accordance to the plan of care.
Coordinate therapy changes to the client's plan of care with the Physical Therapist, Director of Nursing and other caregivers.
Confirm effective coordination of client home care services through timely completion of required documentation and assign medical data towards the office, Director of nursing and other caregivers.
Maintain accurate and comprehensive client medical data.
Instruct clients, family members, and primary caregivers as needed to insure the maintenance of ideal functioning level for each client.
Other responsibilities and duties that may be periodically assigned by the Physical Therapist.
Requirements
Physical Therapist Assistants must have an associated degree from an accredited program and be able to practice physical therapy per state guidelines. Excellent oral, written and interpersonal communication skills as well as organizational and time management skills are required.
