SUMMARY:
The Hospital Liaison - Maternal Infant Program will receive and coordinate program referrals, establish working relationships with local hospitals, and follow up with program participants. Successful candidates will be licensed social workers or licensed nurses. This is a part-time opportunity (10 hours/week) with a day shift schedule.
RESPONSIBILITIES:
- Act as a liaison between hospital units, physicians, and staff to plan for patient discharge and follow up assessments and visits
- Identify families eligible for services and complete initial paperwork based on information from the patient, medical charts, and hospital staff; explain the program and services to the family and obtain necessary signed consents
- Offer the Maternal Infant Health Program (MIHP) services to new Medicaid-eligible clients; coordinates MIHP services to those clients already participating in the MIHP program to assure continuity of care
- Serve as a resource to physicians and staff on the hospital units regarding programs and services offered, as well as other community resources
- Maintain appropriate promotional materials at hospitals for distribution to parents
- Complete weekly caseload of client follow-up phone calls
- Participate in the quality improvement program
- Support the mission, vision, values and key strategies of the organization and development of collaborative relationships with partner organizations
QUALIFICATIONS:
- Bachelor’s degree in social work or nursing required
- Current, unrestricted, licensure in Michigan as a social worker or RN required
- Current, unrestricted, driver’s license required
- Experience in health care, acute care, or human services highly preferred
- Ability to establish positive interpersonal relationships with staff and external community
- Ability to define problems, collect data, establish facts, and draw valid conclusions
- Ability to solve conflict and utilize critical thinking skills
- Ability to maintain a high degree confidentiality and discretion
