GENERAL DESCRIPTION: The Connections Coach will be responsible for the supervision and training of persons served who are participating in Community Connections Program. The focus of services is to assist participants to become more fully integrated into the Community and to learn vocational skills. Primary duties may include community-based training or paid work opportunities.
MINIMUM QUALIFICATIONS: Six month job related experience or education working with people with disabilities. The Ability to obtain First Aid and CPR Certification. Able to communicate effectively and to complete necessary reports. Physically capable of lifting and transferring to assist with restroom use, personal care and physical intervention when necessary to meet the person's needs. This position requires medium work as defined in the Dictionary of Occupational Titles: exerting 20-50 pounds of force occasionally and 10-25 pounds frequently. Must possess an acceptable driving record and be willing to participate in random drug screening as required.
If driving is required for the assignment, must possess a valid driver's license and have an acceptable driving record. Some positions require the employee to obtain a chauffeur's license and a DOT physical within 2 weeks of the assignment. Authorized drivers must be willing to participate in random drug screening as required. As well as posses a minimum insurance coverage of 100,000/300,000.
